To use the Diani web portal available through www.diani.cz, you need to create a personal account using the registration form available on this website. By completing the registration form, creating an account, and accessing the Diani web portal, you agree with the conditions established in this document. The full functionality of the newly created account on the Diani website will only occur once it is activated by the web portal administrator. The administrator has the right to deactivate or refuse the activation of an account created by an external user at any time. For persons under the age of 18, the parent or legal representative is fully responsible for registering and using the system.

By using the Diani system, you acknowledge that this solution is not a certified medical device and is under no circumstances a substitution for medical care, counseling or consultation with a qualified healthcare professional. Always seek advice from qualified healthcare professionals if you have any questions or concerns about your health. Nothing contained on the Diani portal can be taken as part of medical care. Its content and features are designed to help patients, parents, guardians and health care providers better manage diabetes through information transferred to the web portal, their analysis and communication.

Diani provides tools for sharing information between patients, parents, guardians and healthcare providers but transfers to the web portal either through the web application itself or through connected devices, such as the Diabetesdagboka mobile application, activity tracker, glucometer, continuous glucose monitor, blood pressure monitor or weight scale. The user's personal and health-related data may include name, sex, contact details, date of birth, and parameters such as measured blood glucose, drug and insulin doses, carbohydrate consumption, caloric intake, physical activity, sleep activity, weight, pressure, heart rate, HbA1c and notes contained in the diabetes diary mobile application. Diani can also store information about your computer or mobile device such as your IP address, the type of browser you are using, or information about access to a given page of the web portal.

The Diani web portal processes and stores your personal data to provide you with the expected outputs and the functionality available on the web portal. The processor will process your personal data in connection with your membership registration, activity on the web portal and other means of communication, in order to do identification, statistical evaluation, research, education, etc. By sending data to the web portal, you agree that the processor may process the presented data and use it for statistical analysis, clinical research, demographic analysis, profiling of your behavior and characteristics, measurement of your physical condition, and similar processing. Anonymized aggregated data can be exported to any country in the European Union, within or out of the US, for research purposes.


Which types of data do we collect?

The following data can be uploaded to the internal database and eventually displayed on the web portal: the user's registration data and the data that the user automatically or manually transfers to the web portal either through the web application itself or through connected devices, such as the Diabetesdagboka mobile application, activity tracker, glucometer, continuous glucose monitor, blood pressure monitor or weight scale. The user's personal and health-related data may include name, sex, contact details, date of birth, and parameters such as measured blood glucose, drug and insulin doses, carbohydrate consumption, caloric intake, physical activity, sleep activity, weight, pressure, heart rate, HbA1c and notes contained in the diabetes diary mobile application. How can you control your data?

You have the sole right to review the personal data we collect about you. You can do so via email diani@albertov.cz. You have access to your processed personal data. You have the right to ask for information about the processing of your data. In the case that the processing of personal data is in violation of the law, you have the right to ask the processor for an explanation and to remedy this fault. If there is no explanation or remedy, you have the right to contact the Office for Personal Data Protection directly.


What security and protective procedures are used to protect your personal data?

In accordance with actual legislation, we follow reasonable procedures to prevent unauthorized access to personal data and its misuse. We also use security practices, technical and physical access restrictions and secure servers. The access to personal data is only allowed to authorized persons during their work.


Who is responsible for the processing of personal data on company websites and applications?

The processor of personal data obtained through the Diani web portal is Mobilní asistenční systémy s. r. o., with its registered office at Studničkova 7, Prague 2, 12800. This document was created and reviewed on January 4, 2018. The Company reserves the right to make changes to this document without contacting registered members of the Diani web portal.
